It was found in the Sanctuary of Asklepios on the south slope of the Acropolis in two pieces and was reassembled.
On the left side depicted are Asklepios and the goddesses of Eleusis, Demeter, who sits on a small chest, and Persephone with torches in her hands. Towards the three gods, whose cults in Athens were linked together, moves a group of six physicians depicted in a smaller scale as they are mortals.
The five figures’ names and family names are inscribed within the relief olive wreaths on the dedication’s lowest part: [ΘΕ]ΟΔΩΡΙΔΗΣ ΠΟΛΥΚΡΑΤΟΣ, ΣΩΣΤΡΑΤΟ[Σ] ΕΠΙΚΡΑΤΟΣ, ΕΠΕΥΧΗ[Σ] ΔΙΕΥΧΟ[Σ], ΔΙΑΚΡΙΤΟΣ ΔΙΕΥΧΟΣ, Μ[Ν]Η[Σ]Ι[ΘΕΟΣ] [ΜΝ]ΗΣΙΘΕΟ, that is “Theodorides, son of Polycrates, Sostratos son of Epikratos, Epefches sone of Dieuchos, diakritos son of Dieuchos, Mnesitheos son of Mnesitheos”. Three of the names are repeated on the relief’s architrave. All the figures are 4th century BC renowned physicians who, after being honoured by the state with wreaths, dedicated to Asklepios this relief to show their gratitude.
